Please download this attached WAIVER FORM. It is required to be filled out with the player’s info, signed and submitted to Marlon Smith Basketball prior to participating in any of its basketball programs.
*Please note. After you submit your registration, a confirmation email will be send to the Parent/Guardian’s email address. However, if you did not immediately receive this email, please email Marlon directly at marlonsmithbasketball@gmail.com to let him know you’ve registered.
